Find the PERP behind the breakdown.
When people start blaming attitude, communication, or accountability, PERP slows the room down and investigates the hidden system failure causing the damage.
Use this when something keeps going wrong and everyone has a different explanation.
PERP does not hunt for villains. It finds the pressure, missing evidence, failed repair loop, or absent proof that lets the same failure keep reproducing.
1. Name the breakdown.
Keep it concrete. One messy situation. One team, relationship, project, or recurring failure.
Try it with a sample.
Samples are here so the tool teaches itself before you risk a real situation.
2. Interrogate the system.
Use the 0–3 response scale to compare current signals across Pressure, Evidence, Repair, and Proof. This is an Alpha heuristic, not a validated severity measure. Score observable conditions, not intent.
3. Use the repair tools.
These are the sticky pieces. They work in meetings, debriefs, conflict, operations, and relationships.
Meeting Reset
Use when a conversation is spinning.
- What decision are we making?
- What evidence do we have?
- What are we assuming?
- What repair are we choosing?
- What proof will we check?
Incident Card
Use after something goes wrong.
- What happened?
- What pressure was present?
- What evidence was missing?
- What repair is needed?
- What proof confirms change?
Conflict Debrief
Use when people are blaming each other.
- What did each person think was happening?
- Where did interpretation outrun evidence?
- What needs repair: trust, role, process, or capacity?
Proof Check
Use after the fix.
- Owner?
- Date?
- Observable signal?
- What changed?
- What still repeats?
4. Copy the summary.
This is the shareable output. Paste it into email, Slack, notes, or a meeting agenda.
